Scientific articles pertaining to specific disorders are scattered throughout medical subspecialty journals. These children continue to "fall through the cracks" of our medical education system. The increasing recognition that pediatric neuro-ophthalmology comprises a distinct set of diseases from those seen in adults has led to its emergence as a dedicated field of study. "Since the original publication of Pediatric Neuro-Ophthalmology nearly fourteen years ago, interest in the field has burgeoned. Pediatric ophthalmology and pediatric neurology subspecialty conferences often include symposia dedicated to recent advances in pediatric neuro-ophthalmology. Technical advances in neuroimaging have given rise to a more integrated mechanistic classification of neuro-ophthalmological disease in children. Our understanding of neurodevelopmental disorders of the visual system has expanded, longstanding monoliths have been dissembled into component parts, basic molecular mechanisms have taken center stage, and genetic underpinnings have become definitional. Evolutionary alterations can now be observed at the level of the gene, adding a new dimension to our understanding of disease pathogenesis. New classifications now encompass clinically disparate conditions. Descriptive definitions have been supplanted by mechanistic ones, and clinical definitions superseded by genetic ones. Our concept of disease pathogenesis has been revised and in some cases overturned. Bearing witness to these remarkable advancements has compelled me to enhance and expand the first edition of Pediatric Neuro-Ophthalmology into this new and revised one. "In the first edition of this book, our goal was to present the clinical characteristics, diagnostic evaluation, and therapeutic options for the common neuro-ophthalmologic disorders of childhood. Müller Cells in the Healthy and Diseased Retina

Product Description:

Müller glial cells ensheath all retinal neurons in vertebrate retinae. There are a multitude of functional interactions between neurons and Müller cells, including delivery of the light stimuli to the photoreceptor cells in the inverted vertebrate retina, a 'metabolic symbiosis' with the neurons, and the processing of visual information. Müller cells are also responsible for the maintenance of the homeostasis of the retinal extracellular milieu (ions, water, neuro¬transmitter molecules, and pH). In vascularized retinae, Müller cells may also be involved in the control of angiogenesis, and the regulation of retinal blood flow. Virtually every disease of the retina is associated with a reactive Müller cell gliosis which, on the one hand, supports the survival of retinal neurons but, on the other hand, may accelerate the progress of neuronal degeneration:
Müller cells protect neurons via a release of neurotrophic factors. However, gliotic Müller cells display a dysregulation of various neuron-supportive functions. This contributes to a disturbance of retinal glutamate metabolism and ion homeostasis, and causes the development of retinal edema and neuronal cell death. Moreover, there are diseases evoking a primary Müller cell insufficiency, such as hepatic retinopathy and certain forms of glaucoma. Any impairment of supportive functions of Müller cells, primary or secondary, must cause and/or aggravate a dysfunction and loss of neurons, by increasing the susceptibility of neurons to stressful stimuli in the diseased retina.
Müller cells may be used in the future for novel therapeutic strategies to protect neurons against apoptosis (i.e. somatic gene therapy), or to differentiate retinal neurons from Müller/stem cells. Meanwhile, a proper understanding of the gliotic responses of Müller cells in the diseased retina, and of their protective vs. detrimental effects, is essential for the development of efficient therapeutic strategies that use and stimulate the neuron-supportive/-protective - and prevent the destructive - mechanisms of gliosis.

Auditory and Visual Sensations

The subject of the book is the grounding of architectural acoustics in psychophysics (perception, listener preferences) and neuroscience (auditory function, neural correlates of perception and preference). This is the first rational-scientific approach to designing performance spaces that is based on systematic psychoacoustical observations of spatial hearing and listener preferences. Observations of the neuronal correlates of auditory qualities and listener preferences ground the theory of listener preferences in the neurophysiology of the human brain. A correlation-based model of neuronal signal processing in the central auditory system is proposed. Observed psychological and neurophysiological commonalities between auditory and visual sensations and preference patterns are presented and discussed. This book thus spans the disciplines of physics, acoustics, psychology, neurophysiology, and music production, thereby blending science and art.
Temporal and spatial sensations. Temporal auditory sensations include auditory qualities such as pitch, timbre, loudness, and duration, which are modeled using features extracted from the temporal autocorrelation function (ACF) of the sound. Neuronal responses related to these qualities are mainly associated with left hemisphere auditory regions of cerebral cortex. Spatial auditory sensations include the perceived direction of sound, its apparent size (apparent source width, ASW) and its subjective diffuseness deeply related to envelopment. These spatial aspects of sound perception are extracted from the interaural crosscorrelation function (IACF), which takes into account the differences in the sounds that reach the two ears. Cortical neuronal responses related to spatial hearing are mainly associated with auditory regions in the right hemisphere.
Thus, the "primary sensation" evoked by a sound in some space can be divided into two categories: temporal sensations and spatial sensations. Any other subjective responses of the source sound signal and the sound field can be described in terms of temporal and spatial factors. Part I of the book describes experimental results from basic research in acoustics, psychoacoustics, the psychology of auditory preferences, and auditory neurophysiology. Applications include optimal design of concert hall and opera house acoustics, architectural acoustics of speech intelligibility and musical expression, as well as the perception of noise and its annoyance.
In Part II, temporal and spatial sensations in vision are described in terms of similar kinds of correlation-based representations. A seamless, general theory can be established for temporal and spatial aspects of vision that includes subjective preferences for visual environments. Many audio-visual analogies are drawn. A typical temporal sensation of vision is the "pitch" of a flickering light, for which the missing fundamental is perceived in a manner highly analogous to its acoustic counterpart. Visual spatial sensations include contrast, regularity and coarseness. A spatial "vibrato" in a drawing is demonstrated as an application of the theory in the visual arts.
The reader is given a highly fleshed out strategy for architectural acoustical design that is grounded both in psychology and neuroscience. Few other works are as ambitious in attempting to understand and explain such a wide range of perceptual phenomena. This human-centered design approach can be applied to any design practice in which the goal is to optimize the experience of the design product by its users. The proposed theory shows how a unified science of human perceptual experience and preference might be both possible and useful.
